Three Common Presentations of Ingrown Toenails We Successfully Treat

Mild Ingrown Toenail

The nail edge is pressing into the skin but the toe isn't yet infected. There's discomfort and maybe some redness.

Conservative care works well at this stage. The podiatrist skilfully removes the offending nail spike, reshapes the nail edge and shows you how to keep it growing out cleanly.

Most mild ingrowing nails resolve in a single appointment if caught early.

The toe is red, swollen, warm and may be weeping pus or bleeding. Walking is genuinely painful.

We remove the nail spike, drain any infection and dress the wound. You may need a course of antibiotics from your GP if the infection has spread.

Pain relief is usually dramatic and immediate once the offending nail is removed.

About The Treatment

An ingrown toenail happens when the edge of the nail grows into the skin at the side or corner of the toe. It usually affects the big toe. The skin around the nail becomes red, swollen and painful and can develop infection if left untreated. Some ingrown nails settle by themselves but most need a podiatrist to remove the offending spike properly.

The most common causes are cutting nails down the side rather than straight across, tight shoes that crowd the toes, trauma to the toe (stubbing it or dropping something on it) and naturally curved or fan-shaped nails that have a tendency to grow inward. Excessive sweating and some sports activities can also contribute.

Early symptoms are tenderness and slight redness at the corner of the nail. As it progresses you'll see swelling, warmth, throbbing pain especially in shoes and sometimes weeping fluid or bleeding. If it becomes infected you may see pus, the toe may feel hot and you might develop a small lump of inflamed tissue (granulation) at the nail edge.

Diagnosis is straightforward at a clinical examination. The podiatrist looks at the toe, assesses how the nail is growing, checks for any signs of infection and determines whether conservative care or nail surgery is the right approach. Imaging is rarely needed.

We tailor treatment to the severity of the problem:

  • Mild cases: remove the offending nail spike, reshape the edge, dress the area and give you aftercare advice
  • Infected cases: same plus drainage of any pus and possibly GP referral for antibiotics if the infection has spread
  • Recurrent cases: nail surgery (partial nail avulsion) under local anaesthetic to permanently narrow the nail

Most mild cases need just one appointment.

The local anaesthetic injection at the start is the only uncomfortable part and takes about 30 seconds. After that you feel pressure but no pain. Most patients are surprised at how straightforward the procedure is. The toe will be tender for a few days afterwards and we give you everything you need for comfortable healing.

You walk out the same day with a dressing on the toe. Most patients are back to normal walking within 2 or 3 days, back in regular shoes within a week and back to running or sport within 2 to 3 weeks. We see you for a follow-up dressing change within the first week.

It's usually not a good idea. Trying to cut or dig out an ingrown nail at home often pushes a small spike of nail deeper into the flesh, can introduce infection and frequently makes the problem worse. A podiatrist can remove the offending spike in a few minutes safely and properly.

After partial nail avulsion the treated side of the nail is permanently narrower. Most people don't notice unless they look closely. The nail still grows, you can still polish it and it looks essentially normal day-to-day. The procedure is designed to keep as much healthy nail as possible.

For your first appointment, allow up to 60 minutes. This gives us enough time for a thorough history, examination and treatment in the same visit. Follow-up treatments are typically up to 30 minutes for the vast majority of cases. We'll confirm the exact length when you book.

No. You can book a podiatry appointment directly with us with no GP referral needed. We can write to your GP after your appointment if there's anything they should know.

Wear something comfortable that's easy to roll up to the knee. Bring the shoes you wear most often (we'll often look at how they're wearing for biomechanical clues). If you've had any imaging or test results related to your feet, bring those too. Otherwise just bring yourself.
